Expert Consultation (initial)

All patients begin with this one-on-one expert consultation, designed to clarify the clinical picture and provide focused, specialty recommendations.

Includes:  

​

  • Review of relevant medical records

  • Diagnostic video consultation (approximately 60-90 minutes)

  • Clinical synthesis and reasoning

  • Follow-up video consultation with you to discuss recommendations

  • Written report with strategic action plan

  • Discussion with your referring provider, if requested

​

Fee: $3,500 (payment due prior to start of services)

​

This focused consultation may stand alone or serve as the foundation for ongoing care.

Ongoing Care (if needed)

After the initial Expert Consultation, patients may choose to continue services in one of the following ways:

Implementation Program (IP)

Designed to refine or implement the diagnostic or treatment plan developed during your initial Expert Consultation, with the goal of achieving further insight and optimal, lasting results.​ 

 

Dr. Ellenbogen works with you on an ongoing basis to review progress, adjust recommendations, and coordinate care as your plan is developed and carried into practice.

$800 per week
12-week minimum

​

Participation in the program provides the opportunity for up to one hour of Dr. Ellenbogen’s time each week to meet by video, review progress, adjust recommendations, and coordinate care as needed.

​​
Additional time beyond one hour in a given week is billed at $800 per hour.

VIP Access & Implementation

Designed for patients whose who require priority, immediacy, and flexibility, including off-hours availability and accelerated decision-making and direct communication with Dr. Ellenbogen by phone, text, and email.

$12,800 per month retainer for access + $800 per hour
8-month minimum
