We offer two levels of care:
1. Initial Evaluation and Treatment Plan.
This plan is for people who want expert, detailed insight and direction about their health and wellbeing.

All new patients begin with this comprehensive assessment. It includes:
Diagnostic consultation with the medical director — typically a 60–90 minute tele visit.
Detailed review of your medical records
Follow-up visit to present your personalized treatment plan, review results, and outline next steps
Written report summarizing key findings, diagnoses, and personalized treatment recommendations
Many patients use this evaluation as a stand-alone second opinion. For those wishing to continue with Dr. Ellenbogen beyond the initial evaluation and treatment plan, ongoing care is available on an hourly basis at $800/hr. This includes follow-up consultations, record review, correspondence, and related services, all tailored to support your needs.

Designed for leaders, innovators, and high-performance individuals who require rapid access and premium support. The monthly fee serves as a retainer for access, ensuring priority access, ongoing communication, and proactive oversight including some after hours, weekends and holidays. Time spent in consultation, record review, or other professional services is billed separately at $800 per hour.

We do not accept medical insurance as payment for any service. All payments are made directly to the practice at the time of service.
We can, however, provide a detailed receipt for all services rendered. You may submit this to your insurance company for potential reimbursement under “out-of-network” benefits.
This question comes up a lot. And it makes sense for busy or working people. So we are available on weekends and evenings on a case-by-case basis. If that’s your need, we can try to accommodate it, but those slots tend to be reserved for those in the Executive VIP plan. Please mention it when you contact us and we will work with you to get you seen.
Yes, on a limited basis.
In select circumstances, fees may be adjusted based on need and availability. We take special consideration for military personnel, veterans, first responders, and those with financial hardship. Any fee adjustments must be agreed upon in writing prior to the start of care.

t|N|C is a telemedicine practice. In our model, you will be seen by our doctor, just as you’d expect at a traditional clinic. Except, instead of seeing your doctor in person, you see the doctor via secure video conference. It allows you to be seen in your home, or wherever you are, and at your convenience.
Telemedicine is particularly valuable for neurology patients and sleep concerns. For example, some patients with neurological diseases have difficulty commuting. Others wish to share information they might only have at home or even about their living spaces. Others might want a care provider or loved one to join them, sometimes that person will need to join the video conference from far away.
Telemedicine can offer several advantages, including higher efficiency, greater convenience, and more personalized care.
Firstly, telemedicine provides convenience and accessibility, allowing patients to receive specialist care from the comfort of their homes, eliminating travel time; eliminating busy offices with sick patient.
Additionally, telemedicine can lead to quicker consultations and follow-ups, ensuring that patients receive timely medical advice and can monitor their conditions more effectively.
And if the patient wants a care provider or loved one to join from far away, we can easily bring that person into the visit on the video call so we can do it together.
There are two limitations unique to telemedicine to consider.
Firstly, the physical exam. While some parts of an exam can be done by video, others cannot. Contact us and we can help guide whether your situation is right for telemedicine.
Second, telemedicine requires comfort with technology. We will make it easy for you by providing simple instructions. If you can read email, text or visit a website with your computer, that’s most of what you need.
Telemedicine visits also require a stable wifi connection or strong phone network. We encourage you to test your system prior to visit. We can help.
Telemedicine is a medical practice. Dr. Ellenbogen is a board-certified, licensed physician. He can write orders for laboratory tests, imaging, prescription medications — just as any other doctor would.
If you are only doing the New Patient Evaluation, and not planning on continuing care with us beyond that, than the doctor can provide you and your medical team with written recommendations.
If you are going to continue care with us after the New Patient Evaluation, and if you would like us to submit these orders and prescribe medications then we are usually able to accommodate that request.
We take the privacy and security of your personal and medical information seriously, and have several measures in place to protect you.
Adhering to the Health Insurance Portability and Accountability Act (HIPAA), we implement stringent policies to protect patient data. HIPAA mandates that healthcare providers maintain the confidentiality and integrity of patient health information through proper administrative, physical, and technical safeguards.
All electronic medical records are securely encrypted, access to your information is strictly controlled, and all staff are trained in privacy and security best practices. This helps ensure that your information is protected against unauthorized access, breaches, and other potential threats.
Our emails to you, our video conferencing with you and all our forms requested of you are all end-to-end encrypted, which means they are secure in our database, and in transit through the internet.
